Public/Self-Assessment

For public assessments, we collect:

  • Your questionnaire responses
  • Assessment version selected (Adult, Young Person, or Child)
  • Session identifiers (randomly generated)
  • Timestamp of completion

We do not collect names, email addresses, or other personally identifiable information for public assessments.

Clinic Portal

For clinic accounts, we additionally collect:

  • Clinic name and contact email
  • Account credentials (password stored securely hashed)
  • Patient pseudonymous identifiers (clinic-assigned)

2. How We Use Your Information

We use collected information to:

  • Calculate and display your assessment results
  • Generate downloadable reports
  • Improve the assessment tool (aggregated, anonymous analysis)
  • Provide clinic portal functionality for registered clinics

Your data is stored securely on servers with industry-standard encryption. Assessment responses are linked only to randomly generated session IDs, not to any personally identifiable information.
