Public/Self-Assessment
For public assessments, we collect:
- Your questionnaire responses
- Assessment version selected (Adult, Young Person, or Child)
- Session identifiers (randomly generated)
- Timestamp of completion
We do not collect names, email addresses, or other personally identifiable information for public assessments.
Clinic Portal
For clinic accounts, we additionally collect:
- Clinic name and contact email
- Account credentials (password stored securely hashed)
- Patient pseudonymous identifiers (clinic-assigned)
2. How We Use Your Information
We use collected information to:
- Calculate and display your assessment results
- Generate downloadable reports
- Improve the assessment tool (aggregated, anonymous analysis)
- Provide clinic portal functionality for registered clinics

Your data is stored securely on servers with industry-standard encryption. Assessment responses are linked only to randomly generated session IDs, not to any personally identifiable information.
